I hereby testify that all information contained is true to the best of my knowledge.

I DO HEREBY AGREE as a gospel minister to accept the responsibility as a host of an SMTI Video School for the upcoming school year and to oversee the classes at the above-named facility.

I DO HEREBY AGREE, based on my Christian integrity, to honor the requests of SMTI leadership pertaining to the use and application of the SMTI curriculum, administrative policies, and school ethics. I will honor all copyright laws and will not lend any SMTI materials to nonregistered students.

I DO HEREBY AGREE to return all DVDs and/or CDs, as they will remain the property of SMTI in Midland, Michigan.

I UNDERSTAND that one-tenth (1/10) of all tuition received will be returned to the host church to cover facility, administrative, and utility expenses.

I DO HEREBY AGREE to uphold the integrity of the school and to comply to the terms and conditions of this agreement as set forth above.
